The OBBBA requires Medicaid expansion enrollees ages 19 to 64 to report 80 hours of community engagement activities (work, community service, or half-time student enrollment) or demonstrate that they meet the requirements for a qualifying exemption, to maintain their Medicaid coverage.
A CBO analysis of the May 22, 2025, version of the OBBBA found that 4.8 million Medicaid enrollees will become uninsured by 2034 as a result. That number is expected to increase when the CBO releases final estimates of the bill’s impacts.
States with approved Section 1115 waivers may implement work requirements prior to December 31, 2026. The secretary of health and human services may also grant states a one-time compliance exemption until December 31, 2028.
